Job Description:
A company is looking for a Social Worker to join their Case
Management team.
Key Responsibilities
Coordinate care across various healthcare settings while
maintaining confidentiality
Develop unique care plans for members and assist them in navigating
the healthcare system
Assess social determinants of health needs and provide education on
community support resources
Required Qualifications and Education
2+ years of medical social work experience in a healthcare facility
setting
1+ year of care coordination and navigation experience
Master's in Social Work (MSW) from an accredited institution
Licensed Master Social Worker or Licensed Clinical Social Worker is
a plus
Bilingual in Spanish and English is a bonus
